Our Responsibilities

We are required by law to maintain the privacy and security of your medical informationand medical record.

We will notify you if a breach occurs that may compromise your information.
We must follow the duties and privacy practices described in this notice.
We will not use or share your information other than as described here unless you give written permission. You may change your mind at any time.

Your Rights

You have the right to:

Get a copy of your medical record
You may request a paper or electronic copy of your records. We will provide this within 30 days, subject to a reasonable fee.

Correct your medical record
If you believe something is inaccurate or incomplete, you can request a correction.

Request confidential communications
You can ask us to contact you in a specific way (e.g., home or work phone).

Ask us to limit what we use or share
We may not agree to all requests, but we’ll comply when required by law.

Get a list of disclosures
You may ask for a list of times we’ve shared your information for six years prior to the date of your request.

Get a copy of this privacy notice
You may request a paper copy at any time.

Choose someone to act for you
If someone is your medical power of attorney or legal guardian, they can exercise your rights.

File a complaint
If you believe your rights have been violated, contact us at compliance@medshieldascend.com or file with the U.S. Dept. of Health & Human Services.

Bill for services — Submit claims and collect payment from health plans.

With health oversight agencies (e.g., audits, investigations)

As required by law (e.g., court orders, law enforcement)

For organ donation requests

With coroners, medical examiners, or funeral directors

For workers’ compensation or government functions

For research (with IRB approval or your authorization)

To prevent a serious threat to your health and safety or the health and safety of the public or another person

By using our services, you agree to receive appointment reminders, care instructions, and account-related messages via email or SMS. These are not considered marketing and cannot be opted out of.

Marketing Communications
With your separate opt-in, we may send promotional emails or texts (e.g., health tips, product updates). You may opt out anytime by clicking “unsubscribe” in an email or replying “STOP” to a text.

HIPAA Applicability

Due to the nature of our business, the Health Insurance Portability and Accountability Act (HIPAA), the Health Information Technology for Economic and Clinical Health Act (HITECH Act), and similar state laws do not apply to the Medical Group. If there are changes in the applicability of these laws, we will update this notice.

Changes to This Notice

We may change this Notice and its effective date at any time. Updated versions will be posted on our website and made available to you.
